For New Patients
- If you scheduled your appointment online, you will receive an email (different from your appointment confirmation email) with OnPatient information within 48 hours.
- If you scheduled your appointment via telephone, you will receive an OnPatient email shortly after your call.
- Please use Google Chrome browser if available.
The email will include a link that will take you to the OnPatient website. You will be prompted to create a username and password. Please enter the name, date of birth and phone number you shared when setting your appointment.
Fill out the forms as best you can. Instead of filling in your medications and allergies, feel free to just bring a list of these and we will input them for you.
